Impact - Release of Liability 2024

Please complete the steps below to submit our Release of Liability form! As a reminder, this is a TWO STEP PROCESS and both steps must be finished to finalize the process.

INSTRUCTIONS - TWO STEP PROCESS:

• STEP 1 - Sign Up the Student: Please sign up your STUDENT, confirm that all your info is up to date. A separate registration should be made for EACH STUDENT so that it gets digitally filed within the student's profile.  

It will look like you are done...but you're not quite yet!  Just one more step...

• STEP 2 - Fill Out the Release Form: A link for the Release of Liability form will be available after you submit the registration.  It can also be accessed through the link in your confirmation email. Please make sure a PARENT fills out this form.
